Pathways is a program offered to patients with an advanced, life-limiting illness. This palliative approach to care is community-based and offers guidance, support, and education to its participants and family members. There is no cost to the patient for this service. All medical care continues to be directed by the patient’s personal physician.
Making life-changing choices is often a challenge for people with advanced or chronic illnesses. We offer support to help identify and communicate wishes and goals of care. This helpful program gives you the right information at the right time. It is our aim to help you fulfill your life in a dignified and meaningful way.

We offer support to alleviate stress and anxiety. With a tailored program that best fits your interest and needs, the Pathways facilitator can provide life-enhancing education and support on a variety of topics which may include:
Develop or enhance communication skills between family members and healthcare providers.
Address the challenges of living with an advanced or chronic illness. Identify the goals and/or wishes of the patient and develop a plan to achieve them.
Identify stressors associated with the patient’s diagnosis and explore strategies to help the patient and the family effectively manage them. Discuss strategies to reduce anxiety and address anticipatory grief issues as well as interpersonal struggles.
Assist the patient and family in identifying the many practical issues they face and explore tangible solutions. You will gain strategies for handling changes in your daily routine that may occur due to advancing illness.
Assist the patient and family in identifying feelings of anger, loss of control, sadness, fear, and anxiety related to the advancing illness.
Provide an avenue to reconnect or strengthen spiritual beliefs. We assist the patient and family in addressing issues related to the cause and meaning of the disease, and purpose in life and enhancing connection with others.
Offer education and guidance with Living Wills and Medical Power of Attorney.
The Pathways Program is sponsored by Hospice of Midland and its team of professionals is specially trained to guide you through these life-enhancing issues, making Pathways a natural “extension of its mission".
