 A Medical Director oversees patient care given by Physicians, Advanced Nurse Practitioners, and the care teams. The Physicians and Practitioners, along with the Care Team, create a Plan of Care to ensure all patients and families have the resources and support they need.  Each individualized Plan of Care is unique to the patient and their loved ones, to include their input for patient care.

Hospice of Midland Home Hospice Nurse

  Nursing services include in-home visits from Registered Nurses and Licensed Vocational Nurses according to the Plan of Care. During each visit, nurses assess the needs of the patient and family.  Hospice of Midland nurses work closely with the Medical Director and the patient’s Physician to manage pain, medication(s) and symptoms related to the diagnosis.

Hospice Aides preserve the dignity and privacy of patients while providing vital personal care services. These certified workers provide assistance in personal hygiene, including bathing, hair and nail care, shaving, changing linens, and other duties as assigned in the plan of care.

 Hospice of Midland knows that Social Workers are a vital part of helping patients understand the entire hospice care process. Social Workers assist the patient and family in understanding medical insurance, securing legal and financial assistance, offering information for needed community resources, teaching personal coping techniques, and providing counsel to patients and family members. In addition, the Social Worker will assist with Advance Directive Education and aspects of final planning.

 Spiritual well-being is as important to us as the patient's physical and emotional wellness. Let us know how we can be helpful. The Hospice Chaplains respect all individuals' spiritual beliefs and provide guidance and support to the patient, family member, or caregiver(s) who requests it. Such counsel helps patients and families cope with feelings of guilt, anger, frustration, and sorrow. Our chaplains work with the family's clergy when possible.

Hospice of Midland Volunteers provide a tremendous service to our patients and their families. Our volunteers are friendly visitors with a cheerful smile, a listening ear, and a caring heart. They are Hospice of Midland’s hands and feet for the We Honor Veteran’s Program, Hospice Hounds, Pet Peace of Mind, and other programs. They are glad to spend precious time with the patients as their loved one takes needed respite or an opportunity to attend to personal business.

What is Hospice Care?


Hospice focuses on quality of life when you have received a diagnosis of a life-limiting illness. Hospice provides the most effective medical care to control pain and symptoms, offering emotional and spiritual support to patients and those who love them. Patients are treated with dignity; their pain is brought under control and then they can focus on living each day to the fullest.

  • Care team designed to meet each patient’s specific needs and may consist of nurses, social workers, counselors, chaplains, respiratory therapist, dietician, occupational therapist, volunteers and certified nurse aids
  • Medical directors oversee each Hospice of Midland care team
  • Hospice of Midland helps educate families on the community resources available


When is the Right Time for Hospice?

Hospice is not just for the last days of life; it is for the last phase of life. That often begins when a person receives a prognosis that his or her disease process will only worsen and cannot be cured. A patient, family member, physician or may begin the process with a simple phone call. Once a patient is admitted to Hospice of Midland, he or she will be assigned to a care team. These specially trained individuals will visit the patient’s home or current place of residence and assess their needs.


Paying for Hospice Care

Hospice services are covered by Medicare, Medicaid and most private health insurance plans. Hospice of Midland provides care regardless of one’s ability to pay. Neither the patient nor their families ever receive a bill for our services.


Where is Hospice Care Provided?

Hospice is not a place it is a philosophy of care that is delivered wherever the patient calls home. This may include a personal residence, long-term care facility, hospital, assisted living facility, group home or homeless shelter.
