What is Hospice Care?

Hospice at home focuses on quality of life when you have received a diagnosis of a life-limiting illness. Hospice provides the most effective medical care to control pain and symptoms, offering emotional and spiritual support to patients and those who love them. Patients are treated with dignity; their pain is brought under control and then they can focus on living each day to the fullest.


Hospice care includes:

  • Collaboration with the patient’s personal physician
  • Hospice physician services
  • Regular home visits by nurses
  • Assistance from hospice aides for dressing and bathing
  • Counseling services
  • Medical equipment and supplies
  • Pain management and symptom control
  • Spiritual care and support
  • Volunteer support
  • Bereavement programs (individual and group)


What to expect

Hospice of Midland provides a true community of care for patients and their families.

  • Licensed, certified professionals available 24/7
  • Care team designed to meet each patient’s specific needs and may consist of nurses, social workers, counselors, chaplains, respiratory therapist, dietician, occupational therapist, volunteers and certified nurse aids
  • Medical directors oversee each Hospice of Midland care team
  • Hospice of Midland helps educate families on the community resources available


When is the Right Time for Hospice?

Hospice is not just for the last days of life; it is for the last phase of life. That often begins when a person receives a prognosis that his or her disease process will only worsen and cannot be cured. A patient, family member, physician or may begin the process with a simple phone call. Once a patient is admitted to Hospice of Midland, he or she will be assigned to a care team. These specially trained individuals will visit the patient’s home or current place of residence and assess their needs.


Paying for Hospice Care

Hospice services are covered by Medicare, Medicaid and most private health insurance plans. Hospice of Midland provides care regardless of one’s ability to pay. Neither the patient nor their families ever receive a bill for our services.


Where is Hospice Care Provided?

Hospice is not a place it is a philosophy of care that is delivered wherever the patient calls home. This may include a personal residence, long-term care facility, hospital, assisted living facility, group home or homeless shelter.

Palliative Care

Pathways Community Palliative Care is an outreach of Hospice of Midland,

provided at no cost to the patient or family.

Our staff bring a high level of experience and expertise in community-based healthcare. We work in collaboration with your healthcare team, and medical care continues to be directed by your physician.


Our team of Physicians, Nurse Practitioners, Nurses and Spiritual Care Counselors are specially trained to empower you to make informed decisions. We work in collaboration with your healthcare team, and medical care continues to be directed by your physician. 

End-of-Life Care

We offer compassionate end-of-life care services to patients and their families. Our team of experts provides care and support to manage pain and symptoms and provide emotional support during this difficult time.

Grief Support

 We understand that navigating life's challenges can be overwhelming, especially during times of loss. Our dedicated team is here to provide support and guidance through our comprehensive bereavement programs. Discover solace and connection through initiatives like Hope for the Heart, Coffee with Clark, and Surviving the Holidays.
